A Health Home is a readily accessible,
consistent support system for maximizing an individual's health.
Everyone should have a Health Home regardless of an acute diagnosis,
a chronic condition or any health concerns. A Health Home is as
concerned with wellness and prevention as it is with treatment;
and considers the physical, emotional and dental aspects of health.
A Health Home should promote comprehensive and coordinated care
throughout the lifespan, tailored to each person's need.
Essential elements of a Health Home are:
- a central resource for individualized health and wellness
information;
- an ongoing relationship with a trusted health partner who
provides social support and advice on health decisions, and
coordinates all care;
- a central point for collection and coordination of key individual
health information;
- an individualized health plan actively implemented both by
the individual or family and by their health service provider
team; and
- health care that is sensitive to cultural and language issues
and barriers.
